All patients are required to fill out the new patient intake form at the following link prior to their initial visit. Forms can be emailed to DrPalardy@movechirophl.com, or printed at home and given to the Doctor at your initial appointment.
Consent Form
All patients must sign a consent form prior to receiving chiropractic care. The consent form can be found at the following link and should be emailed to DrPalardy@movechirophl.com or printed and given to the Doctor at your initial appointment.
